RELEASE OF PROPERTY DAMAGE LIABILITY
FOR THE INSTALLING OR REPAIRING OF UNDERGROUND SERVICE


It is understood that Berkeley Electric Cooperative, Inc. will install, repair, or replace the underground service at the below-listed location under the following conditions:

A.) Berkeley Electric Cooperative, Inc. will, to the best of its ability, install or repair the underground service wires with as little damage to the landscape as possible.

B.) It is understood that a trench will be dug from the meter base to the feed source for new and replaced underground service wires. If the existing wires are to be repaired, an area will have to be excavated where the existing wires are damaged.

C.) It is the responsibility of the consumer to locate and identify all privately owned underground obstructions.

D.) All trenching and excavating will be kept to a minimum to complete the job required. Berkeley Electric Cooperative, Inc., will backfill all areas excavated at the completion of work. It is understood that these areas will settle over time, and it is the consumer's responsibility to backfill them. For this same reason, the area excavated may be backfilled above grade by Berkeley Electric Cooperative, Inc. if there is enough dirt on site to do so.

E.) Berkeley Electric Cooperative, Inc. will not be held liable in any way for damages to the landscape, shrubbery, trees, unidentified privately owned underground obstructions, etc., which result from the installation or repair of the underground service serving the consumer's property, whose name and signature are listed below.
